This is or has been made an Elephant Hunt subsection. The Elephant Hunt deals with the short comings in systems including: Anti-patterns, code smells and support issues.

A mode of development whereby the voice of a lone wise or expirenced developer is easily drowned by the vote of the inexpirenced or foolish. Also applies to exesive use of "due process" such that end user problems are never acknowledged.
